Better Cardiac Care in reach / outreach services and program management that focuses on health education, promotion, prevention and early intervention of cardiac health issues in Aboriginal and Torres Strait Islander communities.

About your opportunity:

In this position you will:

  • Participate in organising and delivering health promotion, primary health care and secondary prevention endeavours for the cardiology department.
  • Provision of advice and education to the department on how to ensure that services are provided in a culturally appropriate manner. Advocate for appropriate service delivery for Aboriginal & Torres Strait Islander people and identify opportunities for improvement. Participate in planning, policy and governance meetings.
  • Assist with development of culturally appropriate education materials for Aboriginal and Torres Strait Islander patients, families and/or carers.
  • Liaison with relevant Aboriginal and Torres Strait Islander Community Groups and Organisations to improve patient flow, continuity of care and links between services.
