Your new role:
This senior clinical role delivers high-quality social work services within a community-focused, multidisciplinary team supporting patients across the Southern Moreton Bay Islands.
- Provide advanced, evidence-based social work services including assessment, intervention, and care planning in a community setting
- Manage a complex caseload with a high level of clinical independence, contributing to patient-centred outcomes
- Lead and support service improvement initiatives, including quality, research, and development of clinical practices
- Provide supervision, guidance, and education to junior clinicians, students, and support staff
About you:
We're seeking an experienced and compassionate Social Worker who brings strong clinical expertise and thrives in a collaborative, community-based environment.
- Demonstrated high-level clinical skills in social work, particularly in areas such as ageing, chronic disease, and community-based care
- Strong communication and interpersonal skills, with the ability to influence, collaborate, and manage complex stakeholder relationships
- Proven ability to prioritise and manage a demanding workload, including complex case management and discharge planning
- A natural leader who supports and mentors others while contributing to service improvement and innovation
