Your new role:
As a Clinical Nurse within the Chronic Disease team, you will play a pivotal role in delivering advanced, patient-centred cardiac care across community and home-based settings.
- Provide expert clinical leadership in the assessment, planning, and delivery of specialist cardiac nursing care for patients with complex and chronic conditions
- Work autonomously while coordinating care and supporting hospital avoidance and early discharge through innovative community-based service delivery
- Lead and support multidisciplinary teams, including supervision, delegation, and mentoring of nursing staff and students
- Contribute to quality improvement, education, research, and service development to ensure safe, evidence-based, and high-quality patient outcomes
About you:
You are an experienced and motivated nursing professional with strong clinical expertise and a passion for delivering high-quality care in community settings.
- Demonstrated advanced clinical knowledge and critical thinking skills in cardiac or chronic disease management
- Proven ability to work autonomously while providing leadership, guidance, and support within a multidisciplinary team
- Strong commitment to person-centred care, quality improvement, and adherence to clinical standards and best practice frameworks
- Excellent communication, organisational, and problem-solving skills, with a focus on education, mentoring, and continuous professional development

Eight Mile Plains Satellite Health Centre, Maroo-goodji-ba:
The satellite health centre is also known as Maroo-goodji-ba, a name chosen by the community and local Traditional Owners and Elders means ‘peace, wellbeing and place of red earth' in the Yagarabul language.
The satellite health centre treats minor illnesses and injuries that aren't life-threatening and also have a range of specialist services, including kidney dialysis, cancer day therapy and mental, allied health and outpatient services.
