About the role

As a Clinical Pharmacist at The Prince Charles Hospital (TPCH), you will play an integral role in supporting safe, effective and patient‑focused medication management within a large tertiary healthcare setting. This position offers exposure to a rotational clinical environment, where you will work closely with multidisciplinary teams, patients and carers to ensure optimal use of medicines throughout the patient journey.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Deliver clinical pharmacy services [which may include but not limited to dispensing (inpatient, outpatient and discharge), requisitions, medication histories, medication reconciliation, development of Medication Action Plans, pharmaceutical care plans, therapeutic drug monitoring, medicine information and counselling] to multiple Pharmacy work units in accordance with organisational goals.
  • Facilitate the continuum of care through communication with carers, hospital medical staff, GPs, hospital and community pharmacies, nursing homes and case managers to ensure essential information is transferred completely and accurately between hospital and community settings on admission and discharge.
  • Participate in the identification, development and implementation of quality and service improvement activities that enhance the delivery of pharmacy services within The Prince Charles Hospital.
  • Provide clinical education pertaining to routine tasks for students and clinical practice supervision to less experienced staff with the support of a senior clinical staff.
