About us:

Deliver clinical services with a multidisciplinary team focus, incorporating clinical assessment, management/treatment planning, interventions, implementation and evaluation of therapeutic programs and support services aimed at maintaining optimal level of consumer functioning in the least restrictive environment.
About you:
In your new role you will……
- Using a biopsychosocial model conduct mental health assessments and assist consumers and families/ supports to develop a comprehensive individual recovery plan and facilitate the effective delivery, monitoring and evaluation of this plan.
- Work with families and communities to promote recovery and community inclusion and to access generic supports, opportunities and resources.
- Effectively communicate at all levels across the organisation and with external stakeholders, including non-government organisations, consumers and their carers.
