About the role

The Assistant Director Allied Health – Home Hospital is a senior leadership role with responsibility for shaping, leading and advancing a complex portfolio of multidisciplinary, community‑based hospital services across Metro North Health. This position plays a pivotal role in embedding contemporary, safe and effective models of care that support people to receive high‑quality acute, post‑acute and palliative care in their own homes.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Demonstrate expert and comprehensive knowledge of contemporary clinical practice particularly relevant to Home Hospital Team models of care for patients requiring complex care and case management to enhance patient's quality of life, improve functional capacity and avoid unnecessary hospital admission.
  • Provide authoritative counsel and information to the Community and Oral Health Executive and Metro North on multidisciplinary clinical issues relating to acute care in the community, post acute and palliative care.
  • Design, develop and implement innovative multidisciplinary programs, initiatives and standards that ensures efficiency and effectiveness and supports innovation and continuous improvement resulting in sustainable high standards of patient focused care aligning to professional practice standards and legislative requirements.
  • Lead the Home Hospital Service Stream implementation of strategic and service planning in collaboration with service leads and managers and liaison with the and executive team of the Community and Oral Health to support the implementation of initiatives consistent with Allied Health, Community and Oral Health and MNHHS strategic plans and effectively apply as relevant to the service.
  • Demonstrated advanced oral and written communication skills to enable effective interaction in the dynamic inter-health care environment including negotiation, consultation, effective written skills and ability to use management information systems to guide decision making.
