An exciting opportunity exists to join the Cairns Diabetes Centre as a Health Worker – Advanced (Aboriginal & Torres Strait Islander). Reporting to the Cairns High Risk Foot and Diabetes Team, this role works under minimal supervision within a multidisciplinary setting to deliver high‑quality diabetes support, health education, community engagement, telehealth assistance, and culturally safe care to improve health outcomes for Aboriginal and Torres Strait Islander people.

About us:

The Cairns Diabetes Centre is situated at 381 Sheridan Street, Cairns North at the North Cairns Health Precinct.

The Diabetes Centre is the major specialist referral and resource centre in Far North Queensland and provides diabetes services to the Torres Cape, and Cairns and Hinterland Health Service areas.  The team provides both inreach services to Cairns Hospital and outreach or telehealth services to the Cairns Hinterland Hospital Health Service (CHHHS) and Torres and Cape Hospital and Health Service (TCHHS).

The Diabetes Centre applies a multidisciplinary team approach to care in a collaborative practice model.  The team includes medical specialists, nurse practitioner, clinical nurse consultant, diabetes nurse educators, research nurse, enrolled nurses, dietitians, podiatrists, social worker, psychologists,  Aboriginal and Torres Strait Islander health workers, and administration staff.

About your role:

  • Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse.
  • The role may support inreach services to Cairns Hospital and outreach services within CHHHS.
  • Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander High Risk Foot and or Diabetes clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
