Role details

The Operational Services at the Clermont Multi-Purpose Health Service is seeking applications for a Home Care Coordinator to join their team on a permanent basis.

Your opportunity

  • To coordinate, implement, monitor, and review the delivery of flexible in-home support services to eligible consumers with the aim of enhancing their independence.
  • To develop care plans in partnership with consumers and/or carers following My Aged Care assessment of consumer & carer needs and generation & acceptance of referrals to Lagoona Home & Community Care (HACC)
  • To ensure consumer focused services are planned, implemented, coordinated, and delivered in an efficient, effective, and timely manner.
  • To participate & contribute as a member of the local Management team which oversees the day to day running of the Clermont MPHS.

About the Team

Lagoona Home & Community Care operates from the Clermont Multi-Purpose Health Service campus 5 days a week.

Lagoona provides a flexible and coordinated range of practical consumer directed care services, such as Domestic Assistance, Personal Care, Flexible Respite, Community Nursing, Transport, Group & Individual Social Support.

Consumers 65 years and over or ATSI consumers 50 years and over, are eligible for the Commonwealth Home Support Program which they can access by being registered and screened by My Aged Care.

The service is designed to promote consumer independence by doing only those tasks that the consumer is unable to do for themselves, thereby preventing premature admission to residential aged care facilities.

The district covers an area of approximately 90,000km2 and provides services to patients and their families in Bowen, Cannonvale, Clermont, Collinsville, Dysart, Glenden, Mackay, Middlemount, Moranbah, Proserpine and Sarina.
