About the role
Permanent full-time opportunity to contribute to recovery‑focused care, delivering evidence‑based social work services in a multidisciplinary team supporting residents with complex mental illness toward independent living.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide social work assessments, mental state and risk assessments, formulation, and timely social work interventions within identified models of care, including Recovery and Trauma Informed Frameworks.
- Referral and liaison with other healthcare services, government and community agencies including advocacy to promote consumer and family/carer identified needs to support the continuum of care (for example housing providers, Centrelink, NDIA).
- Actively participate in the multidisciplinary team by providing routine Social Work advice (including advice relating to child protection and family violence concerns/family sensitive practice/NDIS) and contribute to the development of individualised treatment plans including discharge planning and service provision.
- Participate in the identification, development, implementation and evaluation of quality, service improvement and/or research activities that enhance and improve consumer care, and contribute to the development of an innovative Mental Health Service.
