About us:
The Cassowary Area Community Health (CACH) Service supports individuals following hospital discharge and those living with chronic conditions who are at risk of hospitalisation. Services include post-acute care, chronic disease management and rehabilitation programs that support recovery and ongoing health and wellbeing.
About your role:
- Coordinate and deliver high-level clinical care and chronic disease programs for individuals and high-risk population groups within the Cassowary Hub, managing a complex and varied caseload.
- Contribute to quality improvement activities, service evaluation, and the development of clinical practices aligned with organisational and professional standards.
- Provide clinical supervision, guidance, and support to less experienced staff and students, as appropriate to level of experience.
