Role details

TheCancer Services at the Mackay Base Hospital is seeking applications for a Clinical Nurse Consultant (Cancer Care Coordinator) to join their team on a permanent basis.

Your opportunity

The Clinical Nurse Consultant – Cancer Care Coordinator (CCC) works across the Oncology Day Unit (ODU) and Medical Day Infusions Unit (MDIU) providing leadership and clinical specialist knowledge and support to ensure consumers and their families experience coordinated and integrated care models.

The Cancer Care Coordinator coordinates the clinical management of Oncology, Haematology and Radiation Oncology consumers within MHHS. This role encompasses a patient-focused and evidence-based practice approach. Coordinating patient access to new and review outpatient clinic consults, supporting telehealth and virtual health appointments, assisting in identifying and reducing consumer barriers to attend treatment appointments and relaying relevant information to key members of the team. Close working relationships with the oncology medical team to assist with managing patients on review lists to ensure timely follow up and care coordination.

The Cancer Care Coordinator role is located at Mackay Base Hospital and supports the service provision to cancer care outreach services at other sites included in the MHHS, whilst also providing leadership and support to other nursing staff within the MHHS.

About the Team

The Cancer Services team is a multidisciplinary team comprising of nursing, medical, pharmacy and allied health professionals. The team provides holistic, evidence-based quality health care for adults across the continuum to achieve quality outcomes for all patients including safe discharge and appropriate follow up care.

The Cancer Services model includes outpatient, telehealth, rapid-access services and day unit treatments.

The ODU and MDIU consists of a total of 12 treatment spaces, providing care to consumers requiring a range of treatments including, but not limited to oncology, haematology, gastroenterology and hepatology.

Our nurses provide education, treatments, support and assistance to patients, carers and other health professionals. The CNC CCC works in close collaboration with the CNC Cancer Services and the Specialist Cancer Care CNCs for Breast, Lung, Melanoma and Prostate Cancer to ensure streamlined pathways for consumers.

We aim to deliver flexible, patient centred service that provides acute & non-acute management for patients and carers with an aim to reduce hospitalisation. Core principals include enhanced patient flow and delivering the right care at the right time in the right setting.
