The Role
An opportunity has become available for a Clinical Nurse (Mental Health - Continuing Care Team) to join Gold Coast Health (GCH) where the focus of our 13 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Undertake and coordinate case management (activities that are designed to help individuals affected by serious and persistent mental illness in gaining access to needed medical, social, educational, vocational, psychosocial, and other necessary services
- Participate in clinical practice supervision with senior staff, as required, and exercise independent professional judgement within their scope of practice
- Contribute to activities that evaluate and ensure the effective functioning of the service such as contributing to strategic planning, reviewing of clinical programs, interventions, outcome data, and key performance indicators as they relate to the CCT service
- Deliver clinical services within the multidisciplinary team in accordance with prescribed professional and ethical standards
About you
To be successful in this position you will require:
- A Bachelor Degree in Nursing
- AHPRA Registration
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Driver Licence
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

- Permanent full time
- Base salary ranging from $113 979 - $122 048 per annum
