This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
Your new Role:
Provide culturally safe support to Aboriginal and Torres Strait Islander patients and families across their hospital journey. You will undertake cultural assessments, advocate for patient understanding, and support discharge planning while working closely with multidisciplinary teams.
The role strengthens links between hospital and community services, improves access and outcomes, and contributes to service improvement and Closing the Gap priorities, supported by accurate documentation and reporting.
About You:
You are a culturally capable professional who advocates for Aboriginal and Torres Strait Islander patients, ensuring culturally safe care, effective communication, and improved health outcomes across the hospital journey.
- Strong understanding of Aboriginal and Torres Strait Islander cultures, health and wellbeing
- Experience supporting patients to navigate hospital and health systems
- Ability to advocate for culturally appropriate, patient-centred care
- Excellent communication and interpersonal skills across diverse stakeholders
- Knowledge of community services, referral pathways and support networks
