About the role
As a Clinical Pharmacist, you will deliver high-quality, patient-centred pharmacy services across a variety of clinical specialties at Redcliffe Hospital and other facilities as required. Working on a rotational basis, you will provide comprehensive clinical pharmacy care including medication reconciliation, dispensing, therapeutic drug monitoring, medication counselling, and the development of medication action and pharmaceutical care plans.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver clinical pharmacy services [which may include but not limited to dispensing (inpatient, outpatient and discharge), requisitions, medication histories, medication reconciliation, development of Medication Action Plans, Pharmaceutical care Plans, TDM, drug information and counselling] to multiple Pharmacy work units in accordance with organisational goals.
- Apply increasingly independent clinical judgement to more complex clinical decision making under the clinical practice supervision or professional guidance of a more senior clinician.
- Facilitate the continuum of care through communication with carers, hospital medical staff, GPs, hospital and community pharmacies, nursing homes and case managers to ensure essential information is transferred completely and accurately between hospital and community settings on admission and discharge.
- Work cooperatively and effectively within the Pharmacy and multidisciplinary teams and with patients/clients
- Participate in the identification, development and implementation of quality and service improvement activities that enhance the delivery of pharmacy services within Redcliffe Hospital.
