We're excited to welcome a Social Worker Senior to the Acute Medical Unit (AMU) within our Social Work Medical Specialties Team at the Sunshine Coast University Hospital.
The General Medicine Social Worker Senior provides high-level social work services to patients within a complex and fast-paced acute inpatient environment, delivering comprehensive psychosocial assessment, intervention, and discharge planning across a broad range of medical presentations.
Key Responsibilities include:
- Coordinating discharge planning across multiple care pathways.
- Navigating aged care and disability systems.
- Facilitating safe and timely transitions of care whilst supporting patients and families in the context of adjustment to illness, palliative and end-of-life care needs.
- Managing complex presentations involving decision-making, capacity, domestic and family violence, elder abuse, and safeguarding concerns.
Remuneration value up to $157 359 p.a., comprising salary between $128 169 - $137 918 p.a., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (HP4).
