We're excited to welcome a Clinical Nurse to Geriatric Evaluation Management and Rehabilitation HITH (GEMR HITH).
The SCHHS HITH service works to alleviate bed pressures, increase patient flow, and improve patient outcomes by delivery of care with a geriatric focus by staff, directly to the patient in the comfort of their own home. The main aim of the service is to maximise function and dignity in the elderly population, using a holistic, comprehensive and inter-disciplinary framework.

Key Responsibilities include:
- Acute Home-Based Intervention: Provide advanced clinical care—including IV medication administration, complex wound management, and blood thinner therapy—to patients in their own homes, maintaining a standard of care equivalent to an in-patient hospital setting.
- Clinical Triage & Suitability Assessment: Evaluate the medical and mental stability of patients for the service, ensuring their home environment is safe for visiting staff and that the patient meets all eligibility criteria for home-based monitoring.
- Deteriorating Patient Management: Utilise high-level assessment skills to monitor common acute conditions (such as pneumonia, cellulitis, or heart failure) and take decisive action if a patient's condition requires escalation back to a physical hospital site.
- Multidisciplinary Liaison: Act as the primary clinical point of contact between GPs, Medical Officers, and Allied Health teams (physiotherapy, dietetics, etc.) to ensure a coordinated, holistic approach to the patient's recovery
Remuneration value up to $4494.75 p.f. comprising salary rates: $57.48 - $61.55 p.h. employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1) (Fixed term temporary part-time position, 64 hrs p.f. for 12 months).
