An exciting opportunity is available for an experienced and motivated Pharmacist Advanced Medicine to join the Pharmacy Department at Bundaberg Hospital. This role is focused on improving patient outcomes through safe and effective discharge medication management and supporting continuity of care across the patient journey. The successful applicant will work a consistent roster pattern of Friday to Sunday each week, supporting weekend discharge activity and contributing to improved patient flow and quality care outcomes.

Your opportunity:
- Provide advanced clinical pharmacy services, professional leadership and operational management to the medical work units and associated multidisciplinary teams within Bundaberg Hospital with a focus on patient flow and criteria led discharges. Clinical pharmacy service provision will extend to supporting continuity of care between facilities of Wide Bay Hospital and Health Service and to stakeholders and customers of Queensland Health in accordance with organisational goals.
- This role requires the successful applicant to work a consistent roster pattern of Friday to Sunday each week including participation in the departments oncall roster.

Our offer:
You will receive a competitive remuneration package value up to $109,020 p.a., comprising salary between $73.13 - $76.30 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP5) (Permanent Part Time, 48 hours p.f.)
