About the role
In this permanent full-time role, you will provide advanced-level social work support to rehabilitation patients, contributing to high-quality clinical outcomes, service improvement, and team leadership within a multidisciplinary environment.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver advanced level Social Work services of a highly-complex and varied nature independently in the rehabilitation setting to patients with complex psychosocial needs and varied health conditions
- Apply evidence-based practice to enable complex clinical decision making and the provision of targeted and timely psychosocial interventions
- Apply knowledge and skills to translate contemporary research into practice and contribute to formal research which develops the knowledge base of the Social Work profession and multidisciplinary team
