Your opportunity
To provide high quality effective and efficient administrative ward clerk and project support to the CaRS Team. This position enables an opportunity of initiative to take ownership where appropriate, work with high levels of communication and have the ability to adapt to a fast-paced evolving work environment.
About the Team
The Care in the Right Setting (CaRs) program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The CaRS Team includes Hospital in The Home (HiTH), Geriatric Emergency Management Rehab-Hospital in the Home (GEMR-HiTH), Residential Aged Care Facility Support Service (RaSS) and Geriatric Emergency Department Intervention (GEDI) care providers. Patients receive quality care that is tailored to their individual health care needs. This is a 7-day service, operating across weekends and public holidays, to ensure continuous care and support for patients in their homes.
The HiTH/GEMR-HiTH service is a hospital substitution service with the expressed aim to provide medical, nursing and allied health care to patients in their home environment. Without this service, eligible patients would otherwise be admitted to hospital for monitoring/treatment in a traditional inpatient bed.
The RaSS model provides care in partnership with RACF's and GP's to improve patient choice of care setting and the quality and safety of care across the care continuum.
