What's involved – This role will lead pregnancy choices predominately the Termination of Pregnancy (ToP) processes and practices across the CWHHS working in collaboration with other healthcare professionals responsible for early pregnancy care provision.
This role will demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for women planning termination of pregnancy within CWHHS; provision of specialist nursing advice; and provide a central point of communication, collaboration, consultation, leadership and education to inter-professional team members in general, relevant to the specific context of this role.
The Nurse/Midwife Navigator (Pregnancy Choices) works across system boundaries to be an advocate, coordinator and facilitator supporting improved health literacy and quality of life, reducing service fragmentation, facilitating improved outcomes, and creating partnerships inter-professionally, cross stream and with consumers to enhance optimal care and model of care effectiveness.
