The Aboriginal and Torres Strait Islander Health Directorate works across all facilities and provides high quality care and support to Aboriginal and Torres Strait Islander patients and their families, as well as cultural education and training for Sunshine Coast Hospital and Health Service staff. The Aboriginal and Torres Strait Islander Health Directorate provides a multidisciplinary team approach to complex care including inpatient support, case management, care coordination through the health service and a supported transition back into primary care.

The Hospital Liaison Officer role functions within the Hospital Liaison Program which supports and works collaboratively with multidisciplinary teams and primary care providers within the Sunshine Coast region to ensure culturally appropriate and safe health services contribute to the improved health and well-being of Aboriginal and Torres Strait Islander patients.

Key Responsibilities include:

  • Facilitate and maintain close liaison links with staff to enhance access to health services and practical social support services for Aboriginal and Torres Strait Islander patients and their families.
  • Develop linkages with all relevant hospital wards, departments, and community services to facilitate admission, discharge planning and referral to follow-up services.
  • Network and liaise between Aboriginal and Torres Strait Islander community organisations, public sector agencies, patients, their families and Aboriginal and Torres Strait Islander Programs within the Hospital and Health Service

Remuneration value up to $121 350 p.a., comprising salary between $77 342 - $106 358 p.a., employer contribution to superannuation (12.75%) and annual leave loading (17.5%) (HFW3). Applications will remain current for 12 months. Please note: only applications from candidates will be accepted (applications that may result in an agency fee will not be considered).

In addition to salary and superannuation, eligible employees may receive a Professional Development Allowance (Category C) in accordance with the applicable Queensland Health certified agreement.

Under section 25 of the Anti-Discrimination Act 1991 (Qld), it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
