About the role:
- Deliver high-quality, person-centred care by conducting comprehensive assessments, planning, implementing and evaluating care for adults with chronic conditions, working closely with patients, families and the multidisciplinary team to achieve optimal outcomes.
- Coordinate care across the continuum by managing services in clinic and community settings, facilitating referrals and collaborating with healthcare providers, while stepping into leadership or shift coordination roles as required.
- Champion safety, quality and clinical standards by adhering to policies and protocols, identifying and managing risks, and actively contributing to quality improvement initiatives within the service.
- Foster learning and professional growth by supervising and mentoring staff and students, delivering patient education, and supporting a positive, high-performing team culture.
About you:
- You hold a Bachelor of Nursing (or equivalent) and current registration through the Australian Health Practitioner Regulation Agency (AHPRA), backed by strong clinical knowledge and a genuine passion for delivering person-centred care.
- You're a natural collaborator who enjoys working in a multidisciplinary setting, contributing to smooth clinic operations and building positive, respectful relationships with patients and colleagues.
- You understand the importance of clinical standards, policies and evidence-based practice, and actively support safe care, quality improvement and continuous learning.
- You bring empathy, integrity and initiative to your work - supporting others, sharing knowledge, and always looking for ways to grow, improve and make a meaningful impact.
What we offer:
Total remuneration value of up to $38,358 p.a. is comprised of:
- An hourly rate between $44.06 - $56.51 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
