About the Team
The CHQatHome service has multiple programs aimed at supporting children to receive appropriate care closer to home.
The service functions as a multidisciplinary team, including nursing, medical, physiotherapy, social work, pharmacy, psychology, dietetics, occupational therapy, speech pathology, psychology, administration and operational officers.
CHQatHome currently supports the following programs:
- Hospital in the (HITH)
- Post-Acute Care (PAC)
- Hospital Avoidance (HA)
- Home Ventilation Program (HVP)
- Tracheostomy in Home Care (TIHC)
- Support of Palliative Care Service
The role of the Support Worker is specifically aimed to support the Home Ventilation Program and Tracheostomy in Home Care Program.
About the Role
The CHQ at Home Support Worker is responsible for the support of a child or young person being cared for within their home environment.
The support worker will be trained and supported to provide safe care to a child or young person with high medical needs within their home or community environment under the supervision of the primary caregiver.
