Hospital in the Home provides direct patient care to inpatients in their home environment. The service provides community-based care for inpatients in their home from Palm Cove to the north, Gordonvale to the south and the back of Lamb's Range to the west.

About you:
In your new role you will:
- Hospital in the Home (HITH) CN is responsible for assessment and support of HITH patients' health literacy.
- Act as an advocate for the clients and client groups to ensure they access appropriate and inclusive health care.
- Provide leadership to effectively plan, manage and evaluate client care activities at an advanced practitioner level within the Hospital in the Home Service.
