Your new role:

As the Clinical Nurse Consultant – Diabetes Unit, you will provide advanced clinical expertise, leadership and consultancy in diabetes education and management across community and home-based healthcare settings. Working collaboratively within a multidisciplinary team, you will play a key role in delivering safe, evidence-based and person-centred care to patients with complex chronic disease needs.

Key Responsibilities

  • Deliver advanced clinical diabetes nursing care, assessment, education and care coordination across the continuum of care.
  • Provide clinical leadership, mentorship and specialist consultancy to nursing and multidisciplinary teams.
  • Lead quality improvement, evidence-based practice and service development initiatives to enhance patient outcomes.
  • Support hospital avoidance strategies through community-based chronic disease management and patient education.

You are a compassionate and experienced nursing professional with advanced diabetes nursing knowledge and a strong commitment to person-centred care.
