An exciting opportunity is currently available for an experienced Social Worker (Frailty Virtual) to join the Social Work Department at the Hervey Bay and Maryborough Hospitals within the Wide Bay Hospital and Health Service. The Wide Bay area is arguably one of the most beautiful regions of Queensland. The Health Service stretches from Agnes Water in the North to Maryborough in the South and provides acute care services to a population of over 225,000 residents including several rural communities.

Your opportunity:

  • The purpose of this virtual service is for subacute and non-acute patients who would benefit from the opportunity to improve their functioning while in their own homes.
  • The position requires working with a specialised team to facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community. This approach aims to reduce prolonged in-hospital patient admissions (extended stays).
  • Deliver high-quality core Social Work services as a multidisciplinary team member to the Wide Bay Hospital and Health Service patients across two hospital campuses and their families and carers to optimise their psychosocial outcomes and achieve maximum benefits from health care services.
  • This position will require you to work across both the Maryborough and Hervey Bay Hospitals, with the allocation of staff being determined by clinical demand.
  • This position reports to the Director of Social Work.

Our offer:

You will receive a competitive remuneration package value up to $53,026 p.a., comprising salary rates $39.87 - $58.60 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 30.4 hours p.f.).
