This role offers the chance to work within a multidisciplinary team delivering patient-centred care to older adults through innovative virtual healthcare models. Your opportunity:
- To deliver high level social work clinical services and expertise to patients identified for the rapid access geriatrician-led model of care, as a multidisciplinary team member.
- This short sub-acute service (up to 14 days) approach aims to reduce prolonged in-hospital patient admissions (extended stays). The position will facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community.
- This position will focus on preventing inappropriate or avoidable admissions by supporting the needs of the patient, their family, and significant others through sub-acute supports in the home environment, to optimise their psychosocial outcomes and achieve maximum benefits from health care services.
- This position reports to the Director of Social Work.

Our offer:
You will receive a competitive remuneration package value up to $39,770 p.a., comprising salary rates $39.87 - $58.60 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 22.8 hours p.f.).
