About the role
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver advanced level evidence-based and discipline specific interventions for allocated older consumers, including a care coordination model utilising evidence-based therapeutic interventions to assist with functional recovery of older consumers.
- Utilise advanced level communication skills to optimise clinical outcomes and work cooperatively and effectively with clients and their families; the multidisciplinary team; other Hospital and Health Services including other mental health services; government and non-government support agencies, General Practitioners, and Private Practitioners.
- Actively collaborate with the Team Leader to support effective clinical and operational delivery, and an innovative team culture, including recruitment, orientation, staff performance and development, giving and receiving feedback, positive problem solving, and supporting the achievement of key performance indicators.
