What your day could look like

As a Clinical Nurse in Community Health, you'll play a vital role in improving the health and wellbeing of individuals and families across the region. From delivering care in home and community settings to working collaboratively with multidisciplinary teams, you'll help create positive, lasting outcomes where they matter most.

Every day, you'll contribute by:

  • Delivering evidence-based nursing care that supports people to live well in their own communities
  • Partnering with patients, families and healthcare teams to plan and coordinate holistic care
  • Identifying opportunities to enhance service delivery and improve patient outcomes
  • Supporting continuity of care through proactive engagement, education and follow-up

You bring both clinical capability and genuine care to your work. You know how to build trust quickly, adapt to changing situations and make thoughtful decisions that put people first. You take pride in helping individuals stay connected, supported and empowered in their own communities, knowing your work has a lasting impact beyond immediate care.

Here's what will help you thrive in this role:

  • Current registration as a Registered Nurse with AHPRA
  • Proven experience delivering community or primary healthcare services
  • Strong clinical assessment, communication and care coordination skills
  • A collaborative mindset with the confidence to work both independently and in a team

You'll stand out from the crowd if you have experience in rural or regional healthcare and/or additional training in community health or chronic disease management. Most importantly, you're committed to improving lives, strengthening communities and embracing the opportunities that come with regional practice.
