About the role
This role supports patients transitioning into Residential Aged Care Homes (RACH) through a unique Interim Care model, delivered via a public-private partnership. You will play a key role in ensuring patients receive the right care in the right setting while longer-term arrangements are secured.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- This Senior Social Worker is positioned within the Long Stay Social Work Team at The Prince Charles Hospital and will be responsible for supporting patients on the Residential Aged Care Home pathway.
- The Prince Charles Hospital has a public-private partnership with a Residential Aged Care Home, where patients receive Interim Care, including Social Work interventions and support to find their long-term Home. This arrangement supports patients to receive the right care, in the right place while long-term arrangements occur.
- This Senior Social Worker will actively contribute to the ongoing service development, evaluation and continual improvement of this new service.
