Temporary Part-Time (64 hours per fortnight) for 3 months (with the possibility of extension)

The Clinical Nurse delivers person-centred, community-based nursing care that supports the health and wellbeing of clients and carers within the Community Care Programs. Working within a collaborative interdisciplinary team, the role focuses on health promotion, chronic disease management, rehabilitation, palliation and early intervention, while considering the social, cultural and environmental factors that influence health outcomes. Strong communication and partnership-building skills are essential to coordinate care and connect clients with the services they need.
