A role that makes a difference
The Clinical Nurse Consultant – Diabetes Educator provides clinical leadership across the Torres Strait Northern Sector of Torres and Cape Hospital and Health Service, supporting diabetes management under the Torres Model of Care.
This role focuses on improving outcomes for people living with chronic disease through coordinated, evidence-based care. You'll work closely with Midwifery Group Practice midwives to support women with diabetes in pregnancy, and provide services across Thursday Island and Outer Island Primary Healthcare Centres to ensure consistent, high-quality care across the region.
