About the role

The HP3 rotational pharmacist role within the Caboolture, Kilcoy, Woodford Directorate (CKW) supports safe and effective medication management across a regional health network. Through structured clinical rotations, the position builds foundational skills, adaptability, and workforce capability while addressing local service needs such as sustainability, coverage, and continuity of care. This role develops flexible, competent pharmacists who deliver high-quality patient care and adapt to evolving service demands, with CKW utilising ieMR across its sites.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Deliver evidence-based routine Clinical Pharmacy services, for example dispensing (inpatient, outpatient and discharge), completing medication histories, medicine reconciliation, clinical review and creating and enacting medication management plans (including TDM), in accordance with organisational goals.
  • Facilitate the continuum of care through communication with carers, hospital staff, GPs, hospital and community pharmacies, residential care facilities, nursing homes and case managers to ensure essential information is transferred completely and accurately between hospital and community settings on admission and discharge.
  • Participate in the identification, development and implementation of quality and service improvement activities that enhance the delivery of pharmacy services within Caboolture Hospital and other facilities.
