About the role
In this role, you will deliver acute medical care to patients in non-traditional settings, supporting a contemporary model that enables patients to receive hospital-level treatment in their homes and community environments. Working as part of the Hospital in the Home (HITH) service, you will manage a varied caseload and provide care across home visits, outpatient clinics and telehealth platforms. You will be actively involved in patient assessment, treatment planning and ongoing review, while collaborating closely with senior clinicians and a multidisciplinary team to ensure safe, coordinated and high-quality care.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Demonstrated skills or interest in developing skills pertinent to General Medicine and Infectious Diseases in the ambulatory acute care setting.
- Provide medical services under the supervision and direction of senior staff.
- Document relevant clinical information accurately, concisely and legibly in patient's charts.
- Assess patient suitability for transfer into HITH at the point of referral, in liaison with senior staff and the HITH multidisciplinary team.
- Conduct interviews, examinations and the identification of clinical problems through a variety of modalities, including home visits, telehealth and clinic reviews.
