About the role
Provide advanced clinical nursing care within the Advanced Heart Failure and Cardiac Transplant Unit, supporting patients with complex cardiac conditions across the transplant and heart failure continuum. Working within the Heart Lung Program, this role focuses on delivering high-quality, evidence-based care to patients with heart failure, ventricular assist devices, and post-transplant needs, within a specialised ambulatory service.
As a Clinical Nurse, you will operate with a high degree of autonomy while providing clinical leadership, education, and support to nursing staff and the multidisciplinary team. You will contribute to complex case management, patient education, and care coordination, ensuring safe, effective, and person-centred outcomes, while supporting service delivery and continuous improvement within a highly specialised clinical environment.
