Your new role:

As a Senior Aboriginal and Torres Strait Islander Health Worker, you will provide leadership and culturally responsive care to support people living with chronic disease. Working within a multidisciplinary team, you will advocate for community needs and contribute to continuous improvement across services.

Key responsibilities

  • Provide clinical and professional leadership to Aboriginal and Torres Strait Islander Health Workers.
  • Deliver culturally safe chronic disease management and patient support.
  • Advocate for improved services and positive health outcomes for Aboriginal and Torres Strait Islander peoples.
  • Participate in quality improvement initiatives and foster a culture of safety and excellence.

  • Strong understanding of Aboriginal and Torres Strait Islander cultures and health priorities.
  • Clinical experience in chronic disease management, including diabetes, respiratory and cardiac conditions.
  • Ability to work collaboratively within multidisciplinary teams and build effective relationships.
  • Leadership skills with experience supporting, mentoring and educating others.

About Chronic Disease:

Chronic Disease is a community based multidisciplinary, multi-sited team which offers education, information and support to adults with complex care and / or unstable or complicated diabetes, respiratory disease, chronic heart failure or following diagnosis of a cardiac disease with a strong focus on hospital avoidance and prevention of hospital re-admissions. The service provides and supports outpatient consultation, group education programs, home visiting and phone consultation services for eligible patients.

Chronic Disease services operate across Brisbane South, Bayside and Logan areas, with service delivery provided in the most appropriate setting for the patient, this could be the patient's domestic residence, community health setting, outpatient settings or as an acute inpatient. The principle objective of the Chronic Disease service is to reduce risk of long-term complications, facilitate early discharge from hospital and reduce the risk of hospitalisation for patients with complex chronic disease conditions.
