About us:

Join an innovative service dedicated to improving care for older people across Central Queensland. The Geriatric Evaluation Rapid Intervention (GERI) and Residential Aged Care Facility Acute Support Services (RaSS) team provides a unique Emergency Department alternative, helping frail older patients receive the right care in the right place while avoiding unnecessary hospital admissions where possible.

Working across both community and hospital settings, you'll support residential aged care facilities through telephone triage, clinical advice, and acute care interventions, while also collaborating with a multidisciplinary Emergency Department team to fast-track assessments, coordinate care, connect patients with community services, and facilitate early discharge back to their home.

About the role:

  • Provide advanced geriatric nursing care through comprehensive assessment, care planning, intervention, and coordination for older patients, including those with complex health needs in the Emergency Department and community settings.
  • Demonstrate clinical leadership and autonomy, making informed clinical decisions, coordinating patient care, leading shifts or teams when required, and supporting nursing staff to deliver high-quality care.
  • Support the care of vulnerable older adults through timely assessment, intervention, care coordination, hospital avoidance initiatives, and facilitating appropriate referrals and discharge planning.
  • Contribute to education, mentoring and workforce development by providing clinical supervision, coaching, and ongoing professional support to nurses and other healthcare workers.
  • Drive quality improvement and evidence-based practice through participation in research, clinical audits, policy implementation, innovation, and continuous improvement activities to enhance patient outcomes and service delivery.

About you:

  • Current AHPRA registration as a Registered Nurse, with a relevant postgraduate qualification in geriatric nursing (or willingness to complete one within an agreed timeframe).
  • Advanced clinical expertise in assessing, planning, implementing and evaluating nursing care for older people in complex healthcare environments.
  • Strong leadership and mentoring skills, with the ability to guide, support and educate staff while promoting excellence in professional nursing practice.
  • Excellent communication and relationship-building abilities, working collaboratively with multidisciplinary teams, patients, families and external stakeholders to achieve positive outcomes.
  • Commitment to quality improvement and person-centred care, using clinical problem-solving, data reporting systems and evidence-based practice to enhance service delivery and patient outcomes.

What we offer:

Total monthly remuneration value of up to $9,772 p.m. is comprised of:

  • An hourly rate between $57.48 - $61.55 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
