About the Unit / Team / Department

The Care in the Right Setting (CaRs) program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The CaRS Team includes Hospital in The Home (HiTH), Geriatric Evaluation Management / Rehabilitation – Hospital in The Home (GEM/R-HiTH), Residential Aged Care Facility Support Service (RaSS) and Geriatric Emergency Department Intervention (GEDI) and 4 hospital funded beds at Glenella Care Residential Aged Care Facility awaiting permanent placement (INTRM). Patients receive quality care that is tailored to their individual health care needs.

The GEM/R-HiTH service provides care to admitted inpatients in their homes or suitable accommodation. This sub-acute service, led by a geriatrician and rehabilitation physician, facilitates multidisciplinary community-based care, enabling a seamless transition home. It supports a range of patients, including those with orthopaedic, stroke, respiratory, and chronic conditions, through short-term rehabilitation aimed at maximising functionality in their own environment. These patients are goal driven, these are set at admission and directs the discharge process and focus. This care is delivered by medical staff, along with nurses, physiotherapists, occupational therapists, social workers, dietitians and speech pathologists.

The RaSS model provides care in partnership with residential aged care facilities (RACFs) and GPs to provide care in nursing homes – thereby reducing preventable Emergency Department admissions and inpatient hospital stays.

Your role (includes but not limited to):

  • Provide medical care under the supervision and direction of senior medical staff within the Care in The Right Setting Unit
  • Ability to recognise your limitations and scope of practice and seek guidance when necessary.
  • Document relevant clinical information and in-patient charts accurately, concisely and legibly. Ensure discharge summaries and other correspondence are completed with minimum delay and that all such correspondence is complete prior to leaving the unit.
  • Arrange investigations, medical treatment and discharge as directed by the assigned consultant.
  • Provide support for junior and senior medical staff as required.
  • Assist with case presentations at teaching meetings where necessary.
  • Participate in clinical meetings and teaching sessions undertaken within the appropriate clinical unit.
  • Participate in continual professional development and medical education.

Vaccine Preventable Disease (VPD) Requirements

It is a condition of employment for this role for the employee to be and remain vaccinated against vaccine preventable diseases as listed on the Role Description.
