Your new role:

As a Registered Nurse within the Rehab@Home team, you'll provide person-centred nursing care to clients in community and residential settings. Working alongside allied health professionals, you'll help patients achieve their rehabilitation goals while promoting independence, recovery and safe care at home.

Key Responsibilities

  • Deliver comprehensive nursing care, assessments and care planning in community settings.
  • Support rehabilitation through wound care, medication management, catheter care and client education.
  • Collaborate with multidisciplinary teams to achieve positive patient outcomes.
  • Contribute to quality improvement, evidence-based practice and ongoing professional development.

About you:

You are a compassionate Registered Nurse who enjoys working autonomously while building strong relationships with patients, families and multidisciplinary teams to deliver exceptional community-based care.

Skills & Experience

  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA).
  • Experience in acute, rehabilitation or community nursing, or transferable clinical skills.
  • Strong assessment, communication and care coordination skills.
  • Current C class driver's licence and commitment to person-centred, evidence-based care.
