About us:
GEMITH is an 8-bed subacute Hospital in the Home service delivering comprehensive geriatric assessment and management in patients' own homes. Caring for people aged 65 years and over, and Aboriginal and Torres Strait Islander people aged 50 years and over, the multidisciplinary team supports those experiencing frailty and age-related physical, functional and cognitive decline, helping them maintain independence while receiving high-quality care in a familiar environment.
About the role:
- Provide ongoing medical assessment, review, and management for GEM-HITH inpatients, including offsite visits and telehealth reviews as required.
- Maintain accurate and timely clinical documentation, including admission, progress, MDT notes, medication charts, and discharge summaries.
- Coordinate safe patient care through appropriate screening, including suitability for admission, VTE risk assessment, and resuscitation planning.
- Support clinical leadership and continuity of care by reviewing referred and Day Hospital patients, managing acute presentations in consultation with senior staff, and keeping the clinical team informed of changes.
About you:
- MBBS or equivalent qualification enabling registration with AHPRA as a Medical Practitioner (with appropriate supervision level if qualified outside Australia).
- Current AHPRA general registration, with supporting evidence including IELTS (or equivalent English language test within the past year) and AMC MCQ (Part 1) completion if internationally trained.
- Demonstrated recency of clinical practice, supported by current professional references.
- Minimum two years' experience in the Australian healthcare system preferred, with experience in Rehabilitation and/or Geriatric Medicine highly regarded.
- Current Basic Life Support (BLS) certification is highly regarded.
What we offer:
Total remuneration value of up to $169,389 p.a. is comprised of:
- A salary rate between $134,489 - $147,585 p.a. (L4 – L7)
- 12.75% employer superannuation contribution
