Your new role:
As a Personal Care Worker with Rehab@Home, you'll play a vital role in supporting older people to remain independent and live well in their own homes. Working alongside nurses, allied health professionals and care coordinators, you'll provide compassionate, person-centred care across community and residential settings, helping clients achieve their rehabilitation and restorative care goals while making a genuine difference every day.
- Provide personal care, domestic assistance and social support to clients in their homes.
- Support clients with medication prompting and daily living activities.
- Accurately document care provided and maintain client records.
- Promote safe manual handling and restorative care principles.

You're a caring, reliable and compassionate individual who enjoys helping others maintain their independence and quality of life. You communicate with empathy, work well as part of a team and are committed to providing safe, respectful and high-quality care.
- Excellent interpersonal and communication skills.
- A compassionate, client-focused approach to care.
- Strong organisational and prioritisation skills.
- Ability to work independently while contributing to a multidisciplinary team.
- Sound understanding of confidentiality, privacy and professional conduct.
