Your new role:

As the Nurse Navigator – First Nations People (Planned Care), you'll provide advanced clinical leadership and care coordination to support Aboriginal and Torres Strait Islander patients throughout their healthcare journey. Working collaboratively with patients, families, community organisations and multidisciplinary teams, you'll help improve access to specialist outpatient services while delivering culturally safe, holistic and person-centred care.

Key Responsibilities

• Coordinate culturally safe, patient-centred care across the continuum of planned healthcare.
• Build strong partnerships with patients, families, community organisations and healthcare providers.
• Provide advanced clinical leadership, care navigation and advocacy for First Nations patients.
• Drive quality improvement, education and service innovation to enhance health outcomes.

About you:

You are an experienced Aboriginal and/or Torres Strait Islander Registered Nurse who is passionate about improving health equity and delivering culturally responsive care through leadership, collaboration and care coordination.

Skills & Experience

• Registered Nurse with current NMBA registration and Aboriginal and/or Torres Strait Islander identified status.
• Advanced clinical experience with strong care coordination and patient advocacy skills.
• Excellent relationship-building, communication and leadership capabilities.
• Commitment to improving First Nations health outcomes through culturally safe, evidence-based practice.

Logan Hospital is the major health centre for one of the fastest growing areas in Queensland. We offer a range of services, including medical, rehabilitation, maternity, surgical and paediatric care.

We offer our staff and students extensive support and training in most medical specialties. As a teaching hospital, we help train future health care professionals and participate in research through strong partnerships with Queensland's leading medical schools, universities, and TAFE Queensland.

Our services will continue to grow as we build bigger and better facilities to support the community. By 2027, we'll have more beds and 5 more birthing suites as part of the Logan Hospital expansion.
