Working across inpatient and outpatient settings, you'll lead assessment, treatment planning, medication titration, education and multidisciplinary care while driving evidence-based practice, quality improvement, research and clinical leadership to improve patient outcomes across CQ.

About us:

The Heart Failure Service delivers specialised, patient-centred care for people living with heart failure, supporting them from hospital admission through to ongoing community-based management. The service combines inpatient education, outpatient clinics, Nurse Practitioner-led medication optimisation, rehabilitation and exercise programs, and regular nurse-led monitoring to help patients achieve the best possible outcomes.

Working closely with the Cardiology Ward and a multidisciplinary team, the service provides seamless, evidence-based care for patients with acute and chronic cardiac conditions, with a strong focus on education, prevention, rehabilitation and improving quality of life.

About the role:

  • Provide autonomous, advanced clinical care for patients with complex heart failure and cardiac conditions across inpatient and outpatient settings.
  • Lead Nurse Practitioner-led heart failure medication titration clinics, optimising treatment to achieve the best possible patient outcomes.
  • Deliver expert assessment, diagnosis, treatment planning and ongoing management using advanced clinical judgement and evidence-based practice.
  • Collaborate with the Cardiac Services multidisciplinary team to enhance patient care, support innovative nursing-led models and improve service delivery.
  • Mentor and support staff through leadership, education and role modelling while contributing to high-quality, patient-centred cardiac care.

About you:

You hold Registration as a Nurse Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA) with a current annual practicing certificate is essential.

Nurse Practitioner Masters level qualification and NMBA endorsement to practice as a Nurse Practitioner is mandatory.

What we offer:

Total remuneration value of up to $15,642 p.m. is comprised of:

  • An hourly rate between $79.47 - $82.97 p.h.
  • 12.75% employer superannuation contribution
  • 17.5% leave loading
