About the role

As a Social Worker within the Cancer Care and Chronic Disease Team, you will provide specialist psychosocial support to patients and families navigating the challenges associated with cancer, chronic illness and complex health conditions. Working across both inpatient and outpatient settings, you will assess psychosocial needs, identify and manage risk, and deliver therapeutic interventions that promote wellbeing, resilience and positive health outcomes.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

• Deliver core clinical social work services to patients affected by cancer and chronic diseases by assessing psychosocial needs of patients, identifying clinical risk and providing clinical interventions to maximise psychosocial outcomes for patients.
• Deliver clinical services to patients and their families impacted by issues including, but not limited to death, grief and loss and adjustment to life threatening illness.
• Provide high level social work services to patients through collaborative participation as a multidisciplinary team member and contribute towards multidisciplinary team processes in relation to service delivery in the allocated clinical area/s.
• Deliver services and support to patients both admitted and within the outpatient setting.
• Provide clinical education about the social work role for undergraduate students, less experienced staff and the multidisciplinary team with the support of senior clinical staff.
