About the role
Permanent part time opportunity within the Hospital in the Home (HITH) and Geriatric Evaluation and Mangement service to apply Social Work skills and knowledge in the delivery of psychosocial care to consumers and their families/carers in the local population to promote, maintain and improve their health, functional ability, and quality of life.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Demonstrate high level clinical knowledge, skills, and expertise in clinical practice standards for Social Work services within a hybrid Inpatient community-based service without the need for direct supervision.
- Provide high level clinical advice regarding Social Work service delivery to diverse stakeholders.
- Provide student education and clinical practice supervision to less experienced staff and students and delegate to the Allied Health Assistants.
- Formulate, implement, and document effective and tailored management plan for complex patients/clients.
