Role details
The Child, Youth and Family Health team at the Carlyle Community Health Centre is seeking applications for a Clinical Midwife (Putting QLD Kids First) to join their team on a temporary basis.

Your opportunity
The Clinical Midwife (Putting QLD Kids First) Program provides targeted, continuity-based care to women and families identifies as vulnerable during the antenatal and early postnatal period.
The role is responsible for early identification, engagement and ongoing support for families with increased needs, bridging care between antenatal services, postnatal inpatient care and Child Health Services to support ongoing engagement with the SHV program and improvement developmental outcomes.
The position delivers care across the continuum from pregnancy through to the early postnatal period, including inpatient support and home visiting up to six weeks post birth, to improve maternal, infant and family outcomes.
About the Team
The Child Youth and Family Health unit is a primary multi-disciplinary health service which is part of the Mackay HHS. The unit provides services for clients 0-18 years of age and their families. The building is located at Carlyle Community Health Centre in Carlyle Street, Mackay. The service extends, offering face-to-face appointments from Mackay to Sarina, Walkerston and areas of the Pioneer Valley and telehealth appointments for the wider Mackay HHS community. The unit offers client focused screening, monitoring, guidance and education along with appropriate referrals and early interventions. The service offers support to the child sector within the Carlyle community health facility as well as offering outreach clinics and home visits. The youth sector is supported within the public high school environments.
