The Sub-Acute Chronic Care Rehabilitation (SACCR) team delivers interdisciplinary, person-centred rehabilitation for people living with complex chronic conditions. By providing proactive, community-based care, the team helps prevent avoidable hospital admissions, supports earlier discharge, and enables people to manage their health closer to home. With collaboration, innovation and clinical education at its core, SACCR also plays an important role in developing the future healthcare workforce through student placements.
About the role:
- Provide advanced clinical assessment and specialist care for people living with chronic disease across Central Queensland.
- Empower and educate patients to better manage their chronic health conditions and improve long-term wellbeing.
- Collaborate with multidisciplinary teams to deliver coordinated, evidence-based care and improve patient outcomes.
- Lead clinical excellence through service improvement, research, clinical governance, and evidence-based practice.
- Mentor and support Nurse Practitioner candidates while fostering education, professional development, and workforce capability.
About you:
- Endorsed Nurse Practitioner with current AHPRA registration and no restrictions to practice.
- Master's degree in Nursing (Nurse Practitioner) qualification with minimum 5000 hours (equivalent to three years full-time) of advanced clinical practice in chronic disease within the six years prior to endorsement.
- Specialist expertise in Respiratory and/or Chronic Wound Care, with a CQHHS-endorsed Scope of Practice.
- Meets NMBA Nurse Practitioner Standards for Practice (2021) and holds, or is eligible for, a prescriber and/or provider number.
- Demonstrate evidence of provision and of coordination of comprehensive contemporary person-centred care through advanced practice skills.
What we offer:
Total remuneration value of up to $187,710 p.a. is comprised of:
- An hourly rate between $79.47 - $82.97 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
