About the role

The Nurse Navigator – Adult Disability is a registered nurse who demonstrates advanced levels in clinical skills, knowledge and translation of contemporary evidence-based care to practice. The Nurse Navigator aims to facilitate safe and efficient care for community patients who are complex by reducing service fragmentation, enhancing patient self-management through understanding of their condition and coordinating the connection to appropriate services to enhance outcomes thus smoothing the patient journey.

Mandatory Requirements:

• Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (Ahpra).
• Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
