About us:
Central Queensland Mental Health, Alcohol and Other Drugs Service (CQMHAODS) delivers specialist mental health, alcohol and other drug care across Rockhampton, Capricorn Coast, Gladstone, Central Highlands and Banana, supporting a diverse catchment of more than 250,000 people.
Within this network, the Older Persons Mental Health Community Service provides specialised, multidisciplinary care for older adults experiencing mental illness across Rockhampton and the Capricorn Coast. The service delivers flexible support across community, home and Residential Aged Care settings, working closely with inpatient, acute care, primary health and community providers to provide holistic assessment, treatment, recovery-focused support and ongoing care.
About the role:
- Deliver advanced, recovery-focused care to older people living in the community with serious mental health conditions, providing assessment, treatment, follow-up and evaluation within a multidisciplinary team.
- Provide comprehensive case management and early intervention, including wellness and lifestyle strategies designed to support recovery, prevent deterioration and reduce re-referral or hospital admission.
- Advocate for consumers and support their families and carers, building strong partnerships with General Practice, Aged Care, NGOs and CQHHS services to ensure coordinated, person-centred care.
- Apply clinical judgement and contemporary practice through critical thinking, problem-solving, multidisciplinary consultation, discharge planning, clinical documentation and outcome evaluation.
About you:
- Bachelor of Nursing or equivalent, with current AHPRA/NMBA registration and annual practising certificate; Authorisation as a Mental Health Practitioner is highly desirable.
- Ability to deliver clinical services to older people, with strong time management, prioritisation and clinical decision-making skills.
- Strong communication and partnership-building skills, with the ability to work effectively within multidisciplinary teams and alongside government, NGO and community providers.
- Commitment to evidence-based practice and contributing to the development of self and others through education, professional networking and shared learning.
What we offer:
Total monthly remuneration value of up to $10,710 p.m. is comprised of:
- An hourly rate between $57.76 - $61.85 p.h.
- 12.75% employer superannuation contribution
