The Patient Access and Coordination Hub (PACH) brings people and information together to make the right decisions, ensuring patients are in the right place, at the right time, receiving the right care.
CQ PACH supports safe, timely care closer to home while improving equitable access to CQHHS services. We respond to changing demand and capacity, use real-time information to guide decisions, and support effective bed management at Rockhampton Hospital.
The Nurse Manager as PACH Coordinator is a key link between clinicians, patients and services. Using advanced clinical judgement, problem-solving and coordination skills, the role supports patient flow, bed access and inter-hospital transfers across the HHS and to tertiary services.
Working collaboratively and autonomously, the PACH Coordinator helps ensure patients receive timely, appropriate and specialist care when and where they need it.
About the role:
- Provide operational and professional leadership across CQHHS, keeping care safe, effective, patient-centred and focused on better outcomes.
- Lead patient flow, capacity management and Inter Hospital Transfers, using real-time information to connect patients with the right care, at the right time.
- Champion direct admission pathways, alternative models of care and local treatment options that reduce unnecessary transfers and improve the patient experience.
- Build strong partnerships across CQHHS, provide expert guidance to rural and remote clinicians, and drive collaboration, accountability and continuous improvement.
About you:
- Registered Nurse with current AHPRA registration and a current annual practising certificate, with the experience and confidence to lead in a complex healthcare environment.
- Calm, clever and solutions focused. You bring advanced clinical judgement, analytical and problem-solving skills, with the ability to make confident decisions when things get busy and time matters.
- A patient-flow pro. You understand access, capacity, demand and inter-hospital transfers, and know how to use data and real-time insights to keep patients moving safely and services performing at their best.
- A natural connector. You communicate, negotiate and collaborate with ease, bringing clinicians, leaders and teams together while inspiring innovation, accountability and continuous improvement.
What we offer:
Total remuneration value of up to $174,208 p.a. is comprised of:
- An hourly rate between $70.92 - $77.00 p.h.
- 12.75% employer superannuation contribution
- 17.5% leave loading
